If i am departing from a U.S. territory port, heading back to a U.S. port, is it necessary to document the last 5 foreign ports on the e-NOA/D ?

I am an experienced Captain. Mainly sailed on Chemical Tankers.

Yes, it is required. But you do not need to key in the details again as you can just edit the last Arrival notice sent before arrival to the first US port.
